It all started...

In 1996, Woodlands LLC owner, Kathy Prasad, acquired AustinWoods Nursing Center. Shortly after, AustinWoods underwent a massive 5-million-dollar renovation. Today, Woodlands at AustinWoods, the former AustinWoods Nursing Center, is among the top most beautiful and inviting rehabilitation and nursing facilities in the Mahoning Valley. Woodlands at AustinWoods was honored to receive recognition as one of the “Best Nursing Homes in Ohio 2015”, as published in the U.S. News and World Report. Additionally, AustinWoods received high ratings in Resident Satisfaction in 2015, alighting them the #1 Rehab and Nursing Center in Mahoning County for Resident Satisfaction.

Understanding the importance of privacy for all residents, AustinWoods offers 102 spacious private rooms for long-term and short-stay residents: all rehab rooms are private. AustinWoods expanded its therapy services to include outpatient therapy by large request of residents and families who received our inpatient services and wanted to continue their therapy at AustinWoods after returning home. As an extra service to our community, Woodlands Outpatient Therapy offers free van transportation to residents who need therapy services and live in the immediate area.


In 2006, AustinWoods was joined by a sister facility, HamptonWoods. During this period of expansion, the Board of Directors created the name “Woodlands” to easily define all services offered through the organization.

Woodlands at HamptonWoods was designed using the “Clustered Cottage Concept”. This concept allows residents with similar needs to live in the same “neighborhood”. HamptonWoods consists of four individual neighborhoods, each having a separate living space and dining area central to the resident rooms. Outside the neighborhoods, residents can enjoy a beautiful living room with fireplace, family kitchen and porch. The porch provides accessibility to a beautifully landscaped secure courtyard, allowing residents safe access to the outdoors. All resident rooms offer private bathroom facilities, most with showers.

Many private rooms are provided at HamptonWoods, and some private rooms offer separate turret space/sitting area. Additionally, companion rooms provide each resident with a private space, separated from their roommate by a built in entertainment unit on each divider wall.

Woodlands Home Care

Woodlands Home Care was also founded in 2006, essentially based on patient and family request. Woodlands Home Care is a medical home care service, providing nursing and therapy services to Mahoning Valley residents. Woodlands has developed an interconnected patient care program within AustinWoods and HamptonWoods. Once home care needs have been identified, Woodlands Home Care team can readily provide family support and transition the client to their home environment. In the home, Woodlands mission is to ensure patients attain positive clinical outcomes in order to achieve their highest level of independence and avoid recurrent hospitalizations. A high percentage of our clients have remained in the home setting, regaining their independence and improving their overall health status. Woodlands Home Care believes in dedication, communication and education of clients, staff and families in order for all to achieve the best that life has to offer.

Woodlands Assisted Living

The vision of Woodlands did not end at HamptonWoods, in 2010 Woodlands LLC opened Woodlands Assisted Living at HamptonWoods. Woodlands Assisted Living offers every amenity to make your lifestyle as independent as possible, yet provides the knowledge that further support is always here for you, if and when you need it. Woodlands Assisted Living is a uniquely designed facility that promotes independence, dignity, privacy, choice and safety while providing 24-hour care and assistance by a compassionate resident care staff and nursing team.

Center for Rehabilitation

With a strong positive presence in the community, Woodlands Owner, Kathy Prasad, has decided to expand Woodlands services, yet again, in response to the growing health care needs of the community. Construction began in the fall of 2015 for Woodlands Center for Rehabilitation, a state-of-the-art 26 bed medical rehabilitation facility. The new building will specialize in short stay rehabilitation and serve patients recovering from surgery, medical procedure, illness, or injury. Woodlands Center for Rehabilitation features 26 beautifully designed private rooms with hotel like amenities and will focus on aggressive treatment plans to help patients regain functional mobility and independence. With the new addition of this facility, the Woodlands at HamptonWoods campus will offer a full continuum of care to the Valley’s residents: short stay rehabilitation, respite care, home health care, out-patient therapy, assisted living and long-term care. The new facility is located just minutes away from the Mercy Health Hospital in Boardman and accessed easily off Route 680, on Western Reserve Road.

Moving Forward

With much pride in the past years of accomplishments, Woodlands moves forward, expanding services to Mahoning Valley residents. Woodlands will continue to evolve the company and its services to meet the ever growing needs and expectations of the community.
